Observation

2b) outside there are shades with pull cords covering the windows. The pull cords are capable of forming a loop and pose a risk. 2c) the hose was seen in the playground strung out. Tape was seen strung out on the floor of the Mt Adams classroom. A cord behind a plant is accessible to the children in St Helens classroom. (3/1/24 the cord has been removed.) An extension cord was seen tapes loosely to the counter, accessible to children. 3/1/24- the extension cord at the counter has been removed. There is an extension cord at the far wall that is in use and accessible to the children. 2d) plastic bags were seen in the classroom, Mt. St. Helens. 3/1/24- the plastic bags have been removed. Outside there were broken plastic items laying the in garden bed area, accessible to children. 3a) The garbage can in the bathroom is cracked. 3/1/24- the garbage can has been replaced. 3f) several tall shelves/cubbies were not securely attached to the wall 3d) a fridge was seen in MT Adams classroom 5b) the outlet in the bathroom is GFCI and TR but the TR appears to be broke as the sheeth is no longer visible on one side of the outlet. An outlet in the mt Adams that teh fridge is plugged into is not TR. 5f) an extension cord is in use in the classroom
